People come in describing a frozen shoulder as tight. Then they mention, almost as an afterthought, that the arm also feels useless. They cannot hold a coffee cup out to the side. They cannot lift a light dumbbell in a range that used to be nothing. That second part is not separate from the stiffness. It is the same problem showing up in a different way.
The shoulder does not just get stiff, it gets quiet
One of the biggest problems with frozen shoulder is that it limits your range of motion, because the deepest portion of the joint glues up. What happens next is the part people are not told: you cannot activate your muscles through a full range of motion. In those ranges they cannot activate, so they atrophy, and they are massively painful.
Think about a road that gets closed for eight months. It is not just that you cannot drive it. By the time it reopens, the surface has degraded and nobody remembers the route. The joint capsule closed the road. The muscles are what happened to it while it was shut.
Why the pain peaks at night
Lying down puts the shoulder in a position it has not tolerated in a long time, with muscles that are both weak and irritable around it. That is why so many people tell us it makes it exceptionally hard to sleep, to lay on it, to live and do things. Night pain is often what finally brings somebody in, more than the stiffness itself.
Turning the signal back up
If the muscles have gone quiet, opening the range is only half the job. Something has to teach the muscle to work in the range you just got back.
That is where neuromodulation comes in. It uses direct current to boost the neurological signal to the tissue so the muscles fire. Picture a dimmer switch that has been stuck low. The bulb works. The signal reaching it has just been turned down by months of pain and disuse. Turning it up while somebody is actually moving means the muscle gets reps in the exact range where it stopped participating.

Most people describe a mild electrical sensation. It runs during the movement rather than instead of it, so you are still doing the exercise while the signal to the muscle is being supported.
